About Individual Support Plans/Individual Service Plans

ISPs should be formed with the involvement of a wide range of people, including parents, group home staff, and representatives of the Arizona Department of Economic Security /Division of Developmental Disabilities (DES/DDD).

The strengths and weaknesses of the person with developmental disabilities should be discussed and the care plan should reflect those strengths and weaknesses. It should include medications, community exposure, matters of hygiene, and plans for improving daily living skills.

The ISP should be followed at all times. Parents have the right to check group home log books to make sure that the ISP is being followed. Staff members who know that parents are checking on them are likely to be more accountable for their actions.
